We’ll coordinate all of your health services, from setting up regular doctor visits and dental appointments to arranging meeting with nutritionists and mental health professionals, to ensuring transportation to all your appointments. In short, we’ll coordinate everything necessary to keep you healthy.
We’ll always be on call to answer your questions, clarify your concerns, and provide you with all the information you need to know about the services available to you. If you need additional services, we’ll coordinate them. If you need more timely access to care, we’ll arrange it. If you need any changes made to the way you receive care, we’ll make sure it gets done.
No matter the nature of your questions, you’ll have access to knowledgeable staff who understand your challenges and will work tirelessly to ensure your needs are met.
Major life changes are hard—but they can be especially overwhelming for I-DD individuals and their families. We help you and your family prepare for life changes and transitions, as well as changes in your needed services, so you can rest a bit easier.
Whether you’re transitioning between levels of care, or after a hospital stay or rehabilitation facility, we’ll set you up with the right services and facilities, aid communication among your providers, and check in with you post-discharge to make sure you are getting the physical and emotional support you need.
When you graduate from school, we assist you with finding employment or enrolling in a day habilitation program. When you turn 18, we help you obtain legal guardianship when necessary.
Your family and support network need the most accurate and up-to-date information in order to be your best advocates and ensure you receive the absolute best care.
We communicate with your family and/or care team to ensure that the Life Plan reflects your needs as well as their preferences, connect them to peer support groups, and give them the proper resources to increase their knowledge, awareness, and engagement in your care.
We help you, your family, and your support network get information about important community-based resources to maximize your quality of life, actualize your goals, and reach your potential.
Whether it’s skills training, finding employment, therapy services, or in-home care, we’ll help identify services, coordinate referrals, and manage any other service. Our ultimate goal is that those we serve gain the most independence, choice, and control over their lives.
