Intake Specialist

Tri-County Care, a well established Care Coordination Organization (CCO), provides Care Management and Coordination services tailored specifically to support individuals with I/DD and their families. Tri-County Care brings together health care and developmental disability service providers to develop an integrated, comprehensive care plan.

We are seeking to hire a an Intake specialist to assist individuals/families to explore eligibility and apply for CCO/HH services. The Intake specialist is the initial contact for agency services.

Essential Responsibilities:

  • fielding inquiries from potential enrollees and referral sources and assisting enrollees with the enrollment process.
  • maintain positive relationships with referral sources and enrollees and respond to any requests or concerns.
  • provide information, education and customer service to Tri-County Care referral sources and potential enrollees.

Requirements:

  • Candidate must be able to multi-task and have excellent communication and organization skills.
  • Minimum of two years experience in working with the Developmentally Disabled (DD) population.
